Even though your pet does not qualify for vaccinations, they are likely still eligible for other services such as dental assessments, spay/neuter assessments, etc. It is at the discretion of medical staff to decline any service for a pet that is too unwell to receive them. You will still be charged the office fee for the preventative care clinic even if your pet is denied services. Administrative services are available Wednesday - Saturday, 8:30 am - 6:30 pm.

Preventative Care:
I understand that the Preventative Care Clinic is run by a Certified Veterinary Technician and is strictly for the listed healthy pet services.
I understand that any concerns I have regarding illnesses, injuries, other medical concerns, prescription medication renewals, doctors consultations, etc. can not be addressed or provided at the Preventative Care Clinic.

Authorized Party:
I acknowledge that I am over 18 years of age, and have the authority to grant consent for this patient to receive preventative care services by Access Veterinary Care staff. I further verify that all given information is true to the best of my knowledge.

Payment:
Payment is due in full at the time of service. Payment plans are not available through Access Veterinary Care, but are available through approved third-party providers. A list of 3rd party providers can be found on our FAQ page or at myaccessvetcare.com/AFR. The paying party is required to pre-qualify for those services before services are provided.

Pet Management:
All pets must remain on a short leash or in a carrier while on property, which includes the parking lot and all common areas. To prevent the transmission of communicable diseases and protect pets and other people from harm, clients are required to manage their pets in a way that reduces contact and interaction with others on Access Veterinary Care property.
Felines: Clients may be refused service or asked to register again another day if their cats are not crated upon arrival. If you do not have a crate, please inform the administrative staff and arrangements will be made. This is required for the safety of the pet and staff.

This is a nonrefundable deposit. The deposit may transfer to another Preventative Care Clinic appointment at Access Veterinary Care, only if appointments are rescheduled through the booking link provided at least 24 hours prior to their original booking date.

Deposit will not transfer:
- If appointment is rescheduled within 24 hours of original appointment time.
- If appointment is rescheduled after the visit date has passed
- For no shows
- For any cancellations made by the client
- If the client is turned away by staff in instances where services requested are outside the scope of the Preventative Care Clinic, there is inappropriate client behavior, mismanagement of pets on clinic property, failure to follow staff's safety instructions, etc.

Clients can receive a copy of their booking link up to 6:00 pm the Saturday before their visit, by contacting the administrative team at 763-390-4050, option 6. Administrative services are closed after that time, and a new link can not be provided before the service date.
